With direct-billing procedures, you are only responsible for the portion of treatment cost that not covered by medical insurance policy.

Please bring your insurance membership card and related identity certification at your first visit. We will make copy and then put them into your medical files. After that, we will prepare necessary medical claim forms for your review during the treatment. You need to sign forms and invoices after your treatment. We will submit them to claim department of your insurer on your behalf. If you are the guarantor for a juvenile client, please state your relationship and sign your name instead.
